I have a v1.6 board that is currently not connected to anything; it is powered with a quality mean well 12v power supply.
The integration into home assistant is made using your example yaml for Esp home (included).
The problem is some of the inputs will randomly toggle on and off.. From my reading, the PCF8574P has an internal weak pull-up, so it should not read a signal unless there's sufficient input on the optocoupler, which would require pulling the input down to ground.
Any ideas what could be causing this?

take a photo of your board, i want to see how you wire.

There's absolutely nothing connected to the board - only power and ethernet.
So these inputs are all floating.

so it's wrong, you have not connect DC terminal, that's why i need to see photo.

I have reviewed the schematic and verified that when I pull the inputs to ground, they do trigger. The board has exactly 12v input, and the INPUT_Dx terminals are floating at 11.6ish, which seems reasonable given the forward voltage drop in the optocoupler.
What DC terminal needs to be connected here?
The terminal not connected would electrically show up identically as an open switch (just with less noise due to the 'antenna' being smaller).

I am *not* using the *outputs*, only the inputs, and the wiring you're showing here is not relevant.

INPUT_4 and INPUT_16 both continue to show this issue - if it is isolated to these, then I'll see if reflowing or replacing the optocoupler on these might make sense.

what power supply you are using? maybe you can replace a new one for a testing.

This is not likely to be the problem here, especially not since it's only these two inputs that seem to misbehaving like this?
